Information
We require a valid prescription(s) written by an authorized doctor for all Rx
Drugs. Once an order has been placed we require that the prescription(s) be
faxed to 1.866.364.9983.

 Mailing cost $10.00 US Funds per order -
Canada Express Post (this is per package, hence you can order multiple items
for this one low shipping price - also, you can save more by ordering together
for your family members and only having to pay one shipping fee of $10.00 US -

When our customers submit their Customer Profiles online they are assured
complete privacy.
1. Please Read the CanadaDrugsOnline.com Customer Agreement and then press I
Agree to continue.
2. Fill in the appropriate fields and be sure to fill in the required ones .
These fields will be marked with a star (*)and must be filled to be valid.
3. Press Submit and your Customer Profile will be automatically sent to the
CanadaDrugsOnline.com Customer Database.
4. Please have your local doctor write your prescription and fax it with your
name Phone number and Address on it.

Download and Fax Customer Profile Instructions
1. Open or Save the Forms package to your computer
2. Print the Forms package.
3. Fill in the blanks.
Just fill in the blank fields and be sure to fill in the ones with a star (*)
beside the fields. These fields are on the forms package. A few pages require
your signature.
4. Please Fax the Following:
a. Completed Forms
b. Copy of Doctor's Prescription
c. Copy of Picture ID (e.g Driver's License or Birth Certificate)

If you don't have a fax machine, ask your doctor's office if they will fax the
forms for you, or use a fax service available at many postal or office services
like Mailboxes Etc. You can also mail the order to us at our mailing address on
our contact us page.
